Polymyalgia Rheumatica and Giant Cell Arteritis (Episode 82, To Your Health with Dr. Jim Morrow)

On this episode, host Dr. Jim Morrow describes two related little-known conditions called Polymyalgia rheumatica and Giant cell arteritis, also known as Temporal arteritis. These conditions involve inflammation of the arteries and usually occur in the elderly and affect mostly women. Dr. Morrow discussed symptoms, diagnosis and treatment, and stressed the need to not wait to get symptoms checked out because of the damage that can occur if left untreated.  He also included an update on the latest COVID-19 booster.

What are giant cell arteritis (GCA) and polymyalgia rheumatic (PMR)?

  • This condition causes inflammation of the arteries in the arms, upper body, and neck.
    • Arteries are blood vessels that carry blood from the heart to the rest of the body.
    • Arteritis is a condition in which the arteries become inflamed (swollen). This reduces blood flow.
      • Giant cell arteritis (GCA) causes certain arteries to become inflamed, red, hot, or painful.
      • It usually affects the arteries above and in front of the ears on both sides of the head (the temples).
      • This type of GCA is also sometimes called temporal arteritis or cranial arteritis.
    • Polymyalgia rheumatica (PMR) is a condition involving inflammation of the muscles in your neck, shoulders, hips, and thighs.
      • This causes stiffness and aching in these areas.
      • It usually develops over time. But for some people, it can start as quickly as overnight.
      • With treatment, PMR usually goes away within a year.
      • It can last for several years in some cases.
      • Both disorders mainly affect people over 50,
        • particularly women.

How are GCA and PMR related?

  • About half of people who have GCA also have PMR. They can have these diseases at the same time, or GCA can develop after PMR.
  • Symptoms of GCA and PMR
    • The symptoms of GCA include:
      • Flu-like symptoms. These can include fatigue, fever, and loss of appetite.
      • Severe headaches.
      • Pain and tenderness in one or both temples.
      • Jaw pain, especially when chewing.
      • Double vision or vision loss.
      • Pain and stiffness in the neck and arms.
      • Unintended weight loss.
    • The symptoms of PMR include:
  • Aching pain and stiffness in the neck, shoulders, hips, and thighs.
  • General muscle weakness.
  • Unintended weight loss.

What causes GCA and PMR?

  • Doctors aren’t sure what causes the inflammation associated with GCA or PMR.
    • Aging and problems with the immune system are possible factors.
    • It also could be related to your genes,
      • or to an environmental trigger like an infection.

Who gets these conditions?

  • You are more likely to develop GCA and PMR if you are older than 50 years of age.
    • The conditions are most common in women between 70 and 80 years of age.
    • For unknown reasons, Caucasian people are more likely to develop GCA and PMR than people of other races.

How are GCA and PMR diagnosed?

  • To diagnose GCA, your doctor will ask about your symptoms and examine the arteries in your temple.
    • He or she will check if they are swollen or tender.
      • They may order blood tests to check for inflammation of the arteries and to rule out other conditions.
        • Often, a biopsy of the temporal artery is needed to confirm the diagnosis.
        • This is an outpatient procedure that involves removing a sample of your temporal artery from your scalp area.
        • To diagnose PMR, your doctor will likely perform a physical exam and ask about your symptoms.
        • He or she may order a blood test.
        • This can help detect inflammation of your arteries and check your blood count.
        • Your doctor could also order a muscle biopsy (removing a small sample of muscle) to check for PMR.
        • Your doctor also can order a c-reactive protein test.
        • This blood test measures the amount of inflammation in your bloodstream but doesn’t identify where the inflammation is located.

Can GCA and PMR be prevented or avoided?

  • There is no known way to prevent GCA or PMR because doctors aren’t sure what causes them.

GCA and PMR treatment

  • Your doctor may prescribe a corticosteroid medicine to treat inflammation and pain.
    • This can help with symptoms of GCA and PMR.
    • These medicines are taken by mouth (in pill form).
    • Once you start taking a corticosteroid, you should feel better quickly.
    • You may need to take this medicine for 6 months up to 2 years.

What else can I do to ease the symptoms of these conditions?

  • Eating a healthy diet and getting enough exercise can help relieve the symptoms of both GCA and PMR.

Living with GCA and PMR

  • Taking a corticosteroid for a long time can raise your blood pressure.
    • It can also increase your blood sugar levels or weaken your bones.
    • Your doctor will need to keep checking you for these side effects during your treatment.
    • Corticosteroids can cause other side effects.
      • These include jitteriness, poor sleep, and weight gain.
      • These symptoms usually get better as you take smaller doses of the medicine.
      • Getting early treatment for GCA is important. If inflamed arteries aren’t treated right away, they could be permanently damaged.
      • This increases your risk of blindness or stroke.
      • Even with proper treatment, GCA and PMR may come back.
        • About half of the time, though, a person who has PMR will experience at least 1 relapse.

Carie Perrino is a Registered Dietitian and Certified LEAP Therapist. During her career she has worked in a variety of places, including hospitals and home health care and developed a love of integrative medicine through her own journey to resolve her IBS.

Carie currently has her hand in many pots:

  1. Registered Dietitian at the Integrative Cancer Clinic with a branch MD Anderson Cancer Center located in Gilbert, AZ. She provides nutrition therapy to cancer survivors and patients who are currently going through treatment to help them focus on wellness and cancer prevention through integrative treatment.
  2. Registered Dietitian at Feel Good Nutrition, LLC, which is her virtual nutrition consulting business. She works with clients with IBS and autoimmune diseases to help them decreases inflammation through tailored anti-inflammatory diets and decreasing exposure to environmental toxins.
  3. Starting a new business with her friend that will be launching this year for pregnant and breastfeeding moms. The company is called My Maternology, LLC – the study of health and happiness from bump to baby. We will be offering a subscription website for nutrition tracking as well as one on one consultations with all professionals relevant to pregnant and breastfeeding moms – dietitian, lactation consultant, health coach, doula and personal trainer. We are hoping to be a one stop shop for relevant, accurate information!

In her free time, Carie enjoys cooking, going to the gym and bingeing on Netflix (while working, of course).

Lynn Lanza is a masters prepared registered dietitian and certified diabetes educator. In her career she had worked in a variety of settings including WIC and public health, college health, oncology and diabetes education and care. She has always had an interest in diabetes, with her masters thesis being a comprehensive prevention program for adolescents at risk for type 2 diabetes in rural southern Illinois.
Lynn currently performs 2 roles:
1. Premise health registered dietitian, Tempe, AZ: providing on site corporate wellness services and both nutrition and diabetes counseling to a corporation with over 1200 employees. Working collaboratively with the physicians assistant for clinical care along with the human relations department, cafe and other food vendors to help create a healthier work campus.
2. Papercranes healthcare, chandler, az: provides nutrition and diabetes education and counseling at family practice owned and operated by her own family. Works collaboratively with nurse practitioners, nurses and other medical staff to provide comprehensive medical care. Lynn has taught various cooking classes and other nutrition and diabetes in the practice’s on site wellness center.In addition, Lynn currently sits on the state of Arizona’s board for the American association of diabetes educators.
In her free time, Lynn enjoys running, teaching body pump at her local gym, swimming, cooking and traveling with her husband, joe.  She is currently growing her family and expecting her first child in June.

WHY BEST OF HEALTH?

I am a Physician Assistant who has been caring for patients for over 20 years and Family Practice (cradle to grave) is my specialty.

Following the footsteps of three generations of family practice physicians (my father, grandfather and great grandfather) this was a natural and genetic calling that I do not regret.

Given this, I have a unique perspective and it’s time to share that perspective through Ask the PA. As a health care provider, teacher, business owner, patient, and advocate for my family and friends throughout the years, it has been painfully apparent there are disconnects within the current state of health care in the US and throughout the world. It can be confusing and frankly very scary.

It is my goal though this series, Best of Health, to bring people together that are passionate about their roles in the business of health care… and yes, whether or not you want to hear that it’s a business, it is — and that’s the fact. I will introduce to you great, dedicated practitioners and people with a passion for getting the word out about their personal journeys and causes. I will provide information about navigating through a confusing system, asking the right questions and share ways for saving money and time along the way.

It’s time to create change by partnering, empowering, and focusing on the patient, their families and their providers of care. It’s time for you to take the driver’s seat rather than leave your health care solely in the hands of entities such as insurance companies and the pharmaceutical industry.

ABOUT BARB

Barb is a Physician Assistant (PA) and a primary care provider with a passion for education and advocacy. Prior to becoming a Physician Assistant (PA), Barb was an accomplished musician and teacher.  In 1992, Barb made the conscious decision to leave teaching and her musical career in pursuit of a career in medicine. She graduated from AT Still University’s first PA class in 1997 with a Masters of Science in Physician Assistant Studies.

Barb has spent several years working in clinics and family practices. Currently, Barb works for Premise Health at Insight Enterprises in Tempe, AZ as a solo practitioner providing primary care to employees and families on campus. She was also a partner in Renaissance Medical Group and Renaissance Medical Properties in Chandler and Maricopa, AZ and had a dual role of Chief Operations Officer and practicing PA for 14 years.

Along with being a PA, Barb is the author of the book Surviving the “Business” of Healthcare, Knowledge is Power! She is a certified Medical Professional Legal Consultant and Patient Advocate, speaker and influencer for change in healthcare for the patient, provider and family members. Barb is also certified by the National Commission of Certification of Physician Assistants and is an active member of the American Academy of Physician Assistants.

In her spare time, she loves to travel, maintaining an active lifestyle that includes running, biking, swimming. She loves to spend time outdoors with her family and friends, whether it’s walking on the beach or hiking in the desert.
