Stop The Bleed Campaign

The ‘Stop the Bleed’ campaign was developed by the American College of Surgeons in 2013 – just a few months after the active shooter disaster that took place at Sandy Hook Elementary School in
Connecticut. ACS convened a joint committee to create a protocol for national policy to enhance survivability from active shooter and intentional mass casualty events. In addition to physicians, this group included representatives from the federal government, the National Security Council, the U.S. military, the FBI, and emergency medical response organizations. This effort served as the
genesis for the Stop the Bleed campaign, which was officially launched by the White House in 2015.
The Stop the Bleed campaign trains, equips, and empowers people to help during a bleeding emergency before professional help arrives – keeping in mind that a person can die from blood loss
within five minutes.  With the support of MAG’s Medical Reserve Corp and other key stakeholders, the Georgia Society of the American College of Surgeons secured a grant from the American College of Surgeons to promote the Stop the Bleed campaign in Georgia. The Georgia Trauma Commission subsequently added one million dollars to its budget to purchase trauma kits for public schools and other public places in the state.

Each Stop the Bleed kit contains a CAT tourniquet, an emergency trauma dressing, compressed gauze, two pairs of gloves, trauma shears, a marker, and instructions.  The Stop the Bleed campaign is also designed to train school nurses and administrators in proper hemorrhage control techniques.

Dr. John Harvey is the medical director of the Medical Association of Georgia’s Medical Reserve
Corps, which is prepared to respond to natural disasters and disease outbreaks in the state. Dr.
Harvey has been a general and trauma surgeon for more than 25 years. MAG’s former president is
the chief of surgery at the Gwinnett Medical Center. He is also a colonel and command surgeon in the
Georgia State Defense Force.
Kathy Browning is the executive director of the Georgia Society of the American College of
Surgeons, which is the largest professional organization for surgeons in the state. She oversees the
organization’s legislative, communications, marketing, and grassroots efforts.
Dr. Shoheb Ali is a resident in internal medicine at Gwinnett Medical Center. As a member of the MAG
MRC, he gave a Stop the Bleed presentation and training demonstration during MAG’s legislative
seminar in June. It is also worth noting that Dr. Ali is the chair of MAG’s Resident Section.

August is Breastfeeding Awareness month in the US and last week was World Breastfeeding week. Today
this program focuses on breastfeeding to lend support to those contemplating and actively breastfeeding
and to raise awareness. Breastfeeding is natural and intimate and can provide a great foundation for
health- but not it may not always be easy. In fact, the first few weeks can be overwhelming. Like many
activities in life, it is a learned skill that can be one of the best gifts babies are given.

According to the CDC’s report card, 79% of moms start breastfeeding after birth in the US. However, many moms don’t
keep it up for the first year, which is the American Pediatric Associations recommended guidelines. The
benefits of breastfeeding are many. On this program, Dr. April Ruffin, a board certified OBGYN physician

This week I hosted Dr. J. Leonard Licthenfeld, Deputy Chief Medical Officer, with the American Cancer Society (ACS). 

We have made considerable progress over the past 25 years in the treatment of cancer. Unfortunately
we still are not able to help enough people survive.  There are many elements that have made this success possible, including research, education, access
and advocacy.

However, we still have a long way to go. Prevention, screening, access to high quality
care, appropriate support through palliative care programs, and investing in research are all important
parts of the cancer continuum.
We have seen exciting advances in cancer treatment recently, with a better understanding of the
genomics of cancer leading to targeted therapies, the introduction of new immunotherapies which have
offered hope where little existed previously, and even newer treatments which have been effective in
treating patients who have failed all other available therapies (such as CAR T recently approved by the
FDA for the treatment of a specific form of leukemia).
All of this comes with a cost: research—including innovative, basic research—has to be supported. We
need to find ways to get more drugs more quickly from the laboratory to the bedside, we have to
wrestle with the cost of new drugs, which can sometimes be so prohibitive that patients and families
can’t afford them, and we have to recognize that not everyone has access to the best and most
appropriate care resulting in disparities throughout our state nation that must be addressed.

In 2016, 7.1M Americans were enrolled in some form of remote patient monitoring program – a 44%
increase in 2016 from the year before. Remote Patient Monitoring (RPM) uses digital technologies to
collect medical data from 1 location and electronically and securely transmits to healthcare providers in
another location for the purposes of assessment and making recommendations. The patient’s HOME is
increasingly becoming a big part of the health care delivery system worldwide.

Benefits of RPM include:  improved patient access, decreased costs and improved healthcare outcomes. RPM is also keeping
patients out of the hospital. Last year, the US had 130M ER visits, but only 9% of these patients were
admitted for care. We are focusing on keeping patients healthier at home and getting interventional care

Discussing Antibiotic Resistant Pathogens

Adrienne Mims, M.D., MPH, FAAFP, AGSF, the vice president and chief medical officer of Medicare Quality Improvement for Alliant Quality, which is Georgia’s Quality Innovation Network-Quality Improvement Organization (QIN-QIO), will discuss antibiotic resistance – “among today’s greatest public health threats” – when she appears on MAG’s ‘Top Docs Radio’ program on the Business Radio-X Network at 12 p.m. today, Tuesday, June 27.

“Antibiotic resistance is responsible for an estimated 2 million infections and 23,000 deaths in the U.S. per year,” says Dr. Mims. “While antibiotics are life-saving drugs that are critical to modern medicine, infections with pathogens that are resistant to first-line antibiotics can require treatment with alternative antibiotics that can be toxic and expensive.”

She explains that, “The most important modifiable risk factor for antibiotic resistance is inappropriate prescribing. It is believed that as much as half of all antibiotic prescribing for upper respiratory conditions might be inappropriate, including antibiotic selection, dosing, or duration, as well as unnecessary prescribing – and at least 30 percent of outpatient antibiotic prescriptions in the U.S. are unnecessary.”

According to Dr. Mims, the Centers for Disease Control and Prevention has released ‘Core Elements of Outpatient Antibiotic Stewardship’ to accompany the existing recommendations for hospital and long-term care settings. Areas of emphasis include…

  1.  Commitment. Demonstrating dedication to and accountability for optimizing antibiotic prescribing and patient safety.
  1.  Action for policy and practice. Implementing at least one policy or practice to improve antibiotic prescribing, assessing whether the policy or practice is working, and making modifications as needed.
  1.  Tracking and reporting. Monitoring antibiotic prescribing practices and offering regular feedback to clinicians or having clinicians assess their own antibiotic prescribing practices.
  1.  Education and expertise. Providing educational resources to clinicians and patients on antibiotic prescribing and ensuring there is adequate access to the expertise that’s needed to optimize antibiotic prescribing.

Dr. Mims also points out that, “Alliant GMCF has received a CMS contract to support the development of outpatient antibiotic stewardship programs in physician offices, urgent care centers, emergency rooms, and federally-qualified health care centers in Georgia.”

And she stresses that, “We will assist clinicians to ensure that every patient receives the right antibiotic at the right time and at the right dose for the right duration, according to today’s evidence-based guidelines.”

Talking Innovations in Treating Pain With Alliance Spine & Pain

On this episode Don and I spoke with Dr. Allen Hord, MD, of Alliance Spine & Pain.  The practice has grown to include 19 offices in several Georgia communities.  The Alliance Spine & Pain website describes their approach to treating pain this way, “In our centers, Alliance Spine and Pain Management physicians focus on non-surgical treatments to treat the root of spine pain — helping people get the relief they need. These spine procedures are clincally proven and follow the strict guidelines of the American Academy of Pain Management. Our physicians are fellowship-trained in interventional pain management, a relatively new medical specialty which focuses on relieving pain through minimally-invasive procedures. Interventional pain management utilizes a variety of techniques to treat conditions associated with chronic pain, including epidural steroid injections, medial branch blocks, facet joint injections, discographic injections, pulse radiofrequency neurotomy and spinal cord stimulation. These procedures are in contrast to higher-cost, often failed, major back surgery and prescription neurotics which often cause unintended physiological and psychological consequences to the patient.

Our state-of-the-art outpatient surgical facilities are approved by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO), the formal accrediting body for healthcare facilities nationwide.

We as an organization also maintain meaningful relationships with hospitals in the area in order to ensure that our patient’s receive the appropriate treatment when medically necessary.

Our practice includes patients covered by commercial insurance, Medicare, and workman’s compensation.
