Azalea Health

This week we started our twice-monthly series with Tanya Mack, CEO of Women’s Telehealth.  Her first guest was CEO and Co-founder of Azalea Health, Baha Zeidan.  Azalea Health is an EMR company that launched in 2008 in Valdosta, GA.  Since then the company has enjoyed steady growth and has been recognized as one of Inc. 500 Nation’s fastest growing private companies, Georgia Top 40 Innovator, and 2014 Emerging Company of the Year Phoenix Award Winner.

Healthcare IT is at the forefront of revolutionizing trends that will change the way healthcare is delivered and patients’ health and wellness outcomes. Healthcare providers and patients who are resistant to technology may be left behind. Today- EMR’s, patient portals, mobile health apps and wearables, cloud computing, and interoperability are not fringe applications but active advances that allow the patients to become more a part of the healthcare team.

Technology makes that possible. Currently, the global EMR market is $11 Billion and the US remains dominant in EMR adoption. As we move to electronic medical records, EMR’s hold a lifetime of health data and can be used to predict our individual health future through modeling.

Today’s guest, Baha Zeidan, CEO of Azalea Health will be discussing his company’s

EMR and Practice management platform and how it is evolving to incorporate these new technology advances.

Vitamin C
Osama Hashmi of Vitamin C

 Vitamin C and OBMedical Company

On this episode of Health Connect South Radio we hosted the CEO and co-founder of Vitamin C and President/CEO of OB Medical Company.  These two companies are creating technology innovations that help two distinct groups, physician practices and expectant mothers in the birthing process, respectively.

Vitamin C’s Osama Hashmi realized that increasing focus on population health management measures meant a need for greater engagement with particular patient groups.  In order to effectively manage chronic diseases such as diabetes or even regular wellness follow up, practices often need to interact with patients to get these visits scheduled.  In many cases, patients won’t take initiative to schedule them themselves.

Hashmi and his colleagues decided to create a platform that interfaces with existing EMR’s that allows providers to ID target populations and create a variety of pre-created content for distribution to patients.  In some cases, it’s a pre-recorded automated phone call that can encourage the scheduling of a follow up visit.  In others, it may be an automated call that queries the patient for particular data such as vitals or blood sugars, quantity of exercise, etc.

In still others, it may be a text or email generated that goes to the patient.  In this way, the providers are able to more effectively “scale” their resources to be able to reach what is often hundreds or even thousands of patients with necessary contact to facilitate follow up.  The end result is better patient outcomes as well as increased revenue for the practice due to scheduling of necessary, yet high-Revenue visits (based on time/acuity).

OBMedical Company saw an opportunity to improve upon monitoring technology used in the OB delivery room.  As I spoke with President/CEO, Weaver Gaines, he explained the technology for the fetal heart rate, contraction frequency/intensity, etc. have not really changed in roughly 40 years.

Today, mothers are tethered to a monitor by wires leading to sensors that are held in place by elastic velcro straps that encircle the mother’s abdomen.  They are challenging to keep in position and can provide limited accuracy in cases where the mother is particularly obese.

OBMedical Company has designed a monitoring device that uses wireless electrodes that measure electrical current through the patient’s skin rather than having to rely on skin tension.  This allows the mother to be able to ambulate in the room during labor while continuing to be monitored.  Additionally, it allows for greater accuracy when the mother is obese.

Health IT and Accelerating Innovation

This week I sat down with experts from the healthcare IT sector.  John Mamon is the CEO of mPoweredIT, an outsourced IT company that devotes expertise to the small to mid-sized medical practice, helping them manage their computer hardware, infrastructure, and software needs for far less than it typically costs to employ someone dedicated to these functions within the practice.

John talked about how often, in an effort to save money, small practices place responsibility for managing these facets on a practice manager, a clinician within the office, or even the doctors themselves.  Clearly this can take away from the core function of the office—taking care of more patients.

He shared a number of important concerns that can expose these offices to risk such as HIPAA requirements, downtime of equipment, ACA compliance, and others.  Each of these can create significant cost or loss of productivity when small offices attempt to handle things on their own.

mPoweredIT is able to scale their services to the needs of the practice, providing the full suite of IT management or taking over particular elements on behalf of the practice.  The company also partners with several other health experts who can provide additional support in areas such as billing/revenue cycle management, EMR selection, and others, to allow their clients to maximize productivity and profitability at the same time.

John introduced us to Harvey Shuster, of Emergence, a Georgia Medical Accelerator.  Harvey, a multiple-time entrepreneur, was one of the founders of Peachtree Software among others.  He and his team work with emerging healthcare technology companies, helping them to commercialize and bring to market their devices/technologies more quickly.

Harvey brought along Osama Hashmi, CEO/Founder of VitaminC.  This company adds an additional layer of automation and engagement to EMR systems.  Their software allows providers to contact patients and encourage follow-up visits where appropriate to facilitate care/outcomes while at the same time helping the practice to maximize its potential for seeing patients in need.

VitaminC integrates well with numerous EMR systems and has proven valuable to primary care doctors and specialists such as dermatologists, plastic surgeons, and others.

Mark Janiszewski of Greenway Health and Diana Keough of ShareWIK

Greenway Health

A couple of topics that have come up repeatedly over the course of weeks since we started the Health Connect South Radio show:  Population Health and Interoperability.  On this week’s show we sat down with health IT expert, Mark Janiszewski, EVP of Product Management for Greenway Health.

Mark shared how the company has recently introduced new solutions that are aimed at facilitating better population health management initiatives.  Recent changes in the law are placing greater requirements for focus on preventive health and patient outcomes over the older disease-focused models that reimbursed on a fee-for-service basis.

Greenway’s platform gives providers access to data that empowers them to close gaps in care delivery and better manage a population’s high-cost problems such as diabetes, heart disease, and other chronic illnesses.  Additionally, the company is providing a portal called, Greenway Exchange, that gives patients ability to see their health data, schedule appointments, and even interact with their care providers.

We talked about how the company helps their clients prepare for the coming implementation of ICD-10 coding requirements that are currently set to go into effect in October 2015.  The new coding format will add a much greater degree of detail in documenting patient problems and treatments.

The goal of the ICD-10 changes is to facilitate research, improve outcomes through better case management, decrease medical errors, enhance public health, educate patients on costs/outcomes, and more.

Mark shared some of the challenges that health organizations face in meeting the expectations for the ICD-10 documentation.  The company is helping to prepare their clients for the upcoming changes in coding.

“Greenway Health has always worked to empower our customers so they can deliver high-quality care while being financially sound,” said Tee Green, CEO of Greenway Health. “Never has that been more important given the rise of pay-for-performance, healthcare consumerism and a population with an increasing number of chronic conditions that requires coordination between multiple caregivers. Greenway Community enables providers to gain actionable insight into clinical and financial performance at all levels of the care continuum like never before.”

Greenway Health has been providing health IT solutions since 1977.  Over time, the company has grown significantly, both organically and through merger.  As an enterprise, the company delivers clinical, financial, and administrative information tools providers need to practice medicine the way they want to practice.

Greenway helps providers stay ahead of the complex regulatory requirements, maximize revenue and, most importantly, focus on their patients rather than on technology.  The company accomplishes this through intuitive user interfaces, interoperability leadership to exchange clinical data and facilitate connected care, and a grass-roots service culture.

Medical Association of Georgia

Senate Bill 158

On this week’s show we continued our monthly series with the Medical Association of Georgia.  MAG’s CEO/Executive Director, Donald J. Palmisano, Jr. stopped by for a discussion on the state of Senate Bill 158, changes in the insurance contracting arena, and other topics on the MAG agenda.  Donald also shared how he and a colleague were able to raise over $40,000 to support the Think About It campaign to raise awareness and fight prescription drug abuse/addiction in Georgia, participating in a 100 mile race and completing it in under 24 hours.  In the days since Donald joined us on the show, Senate Bill 158 was passed.

MAG continues to take steps to enhance the relationship between health insurer Blue Cross Blue Shield of Georgia, Inc. (BCBSGa) and physicians in the state.

At the end of 2014, Georgia Insurance Commissioner Ralph Hudgens rescinded physician contract amendments that BCBSGa had put into place in Georgia following “numerous complaints from physicians (and their practices).”

First MAG/BCBSGa “Physician Advisory Group” meeting took place on February 12. It is forum for physicians to express their concerns, it will give Blue Cross the opportunity to disseminate information about new payer initiatives, it will be a venue for improving communications, and it will serve as a mechanism for physicians to weigh in on Blue Cross’ clinical policies, operations and contracting practices.

The advisory group consists of four MAG member physicians as well as MAG Health Policy and Third Party Payer Advocacy Department Director Susan Moore and BCBSGa Senior Clinical Officer Mark Kishel, M.D., and BCBSGa Director of Network Management/Georgia Provider Solutions Hayden Mathieson. They will meet a minimum of three times a year.

Rental Networks

MAG supports legislation that would limit rental networks in Georgia. A rental network involves a health insurer that rents or sells its network of physicians to another health insurer. These second-level insurers then include the physicians in their health insurance plans – and pay the physicians an even deeper discount – even though they don’t have a contract with the physicians.

It’s not uncommon for physicians to offer their services to a health insurer at a discounted rate because the higher patient volume offsets the costs. However, the aforementioned insurers rent or sell their networks without the physician’s knowledge – so the physicians often aren’t aware that they are in a given network or that they are contractually obligated to deliver patient care at a greater discount.

Rental networks result in mass confusion and higher administrative costs (e.g., the additional staff time that is required to verify a patient’s health insurance coverage and/or confirm the proper payment). Rental networks can also reduce the accessibility of care because physicians are forced to accept the lower (i.e., “re-priced” or “re-rented”) payment or refuse to see the patient for any follow-up or future care.

Rental networks are an inappropriate, profit-driven tactic that undermines the practice environment in Georgia that will exacerbate the physician shortage in the state. Sixteen states have now adopted laws that regulate or limit or prohibit rental networks.

Insurers are prohibited from using rental networks in federal employee health benefits plan contracts.

All-Products Clauses

Insurance companies use ‘all-products or all-or-nothing’ clauses as a cost-control tactic to force physicians to participate in every health insurance product that they offer or be blocked from caring for patients in the insurer’s plan altogether.

By forcing physicians and their practices to agree to all-products clauses, health insurers are undermining the economic viability of the medical profession in Georgia – keeping in mind that a report that was prepared by IMS Health for the American Medical Association determined that physicians in Georgia “created a total of $29.7 billion in direct and indirect economic output (i.e., sales revenues) in 2012… [and] each physician supported $1,559,494 in [economic] output.” It is also worth noting that the report found that “…physicians supported 205,869 jobs (including their own)…[and] $1,089.6 million in local and state tax revenues in 2012.”

Because physicians are constantly wrestling with these manipulative contract provisions, they have less time to spend with their patients.

All-products clauses violate several individual rights, including the right to contract and an owner’s right to operate a business in a free and autonomous way.

Physicians and medical practices in Georgia should be free to accept the health insurance products of their choice versus the ones that’s imposed on them by profit-driven insurance companies.

Eleven states have enacted prohibitions on all-products clauses, including Alaska, Arkansas, Florida, Indiana, Kansas, Kentucky, Maryland, Massachusetts, Minnesota, Ohio, and Virginia – as well as Washington, D.C.

Patient Engagement

This week I caught up with our friend, Frank Martin, of The Medical Consultants Group.  We talked about how his consulting firm can help a wide range of medical practices, from solo offices to multi-site large groups.  Frank talked about the variety of facets of a business that is a medical practice improve operations.

Frank talked about strategies for helping patients related to managing their out-of-pocket obligation, including deductible and co-payment.  We also discussed how the ACA has placed new emphasis on patient satisfaction and outcomes relating to how the practice (or isn’t) reimbursed.  Another requirement is to provide an electronic portal that allows remote access to patient data BY the patient, coupled with a means to transmit/receive relevant information to/from patients.

We discussed the difference between legacy EMR systems (where software was installed on hardware in the office) and cloud-based applications where data and the software exist on a remote cloud-computing-based architecture.  The decision-making capability of the cloud-based applications, coupled with security and rapid access to important data, make modern cloud-based EMR applications the wise choice for practices to convert to today.  These modern applications do not require on-site updates of the application on every device.  Now, the application is updated where it lives, meaning the user’s experience is not interrupted or inconvenienced.

Frank places great emphasis on helping his client practices become better-running businesses.  To that end he works with the group to be able to change processes, or software, business plan, and/or other key facets such as office location and office space leasing to operate more efficiently and profitably.

Diabetes Prevention Planning Team

According to the CDC, as many as 28 million Americans have diabetes.  And as many as 1:4 of them do not realize they have it.  That means today, potentially almost 10% of the US population are dealing with this chronic disease that puts them at risk for a myriad of health problems (many of them deadly), including heart disease, stroke, kidney disease, blindness, limb loss due to amputation, poor healing after surgery, greater risk for infections, and more.  It’s clearly a serious problem.  And, with an ever-growing number of obese persons in the country, including a concerning number of young people, these statistics are likely to grow.  In light of these sobering facts, a number of key players within the community from government agencies to health care providers, and innovative companies are beginning to put significant effort into preventing diabetes.  In Atlanta, the Diabetes Prevention Planning Team is a group of forward-thinking executives, health experts, and political officers who are joining together to do just that.

This week we were joined in studio by Dr. Luci Ide, CEO of Rimidi Diabetes, and Karl Smith, PhD, Project Manager of Georgia Campaign for Adolescent Power and Potential (GCAPP).  Rimidi Diabetes is a health technology company that has developed and launched an application that enables diabetes patients to track and share key health-related data points with the clinicians guiding their care.  With this information, the patients and providers are able to better monitor glucose levels and other indicators and determine how behavioral and medical treatment changes are impacting the patient’s condition.  Luci is one of the team members for the Diabetes Prevention Planning Team and she talked about how increasing awareness of the disease and how it can be prevented are important facets of combating diabetes.  She used the analogy of a bath tub, where how quickly it fills is determined by how much water flows out of the drain (patients dying as a result of the disease) versus how much is flowing in from the faucet (new diabetes patients).  She went on to discuss the fact that today, more patients are living longer and managing their diabetes better than in the past.  Our next challenge is to begin to decrease the number of new patients developing diabetes in populations where it can be prevented.

HCS3

Karl Smith, also a member of the Diabetes Prevention Planning Team, shared how GCAPP is working to engage faith-based organizations in Atlanta communities that have populations that are at a high genetic risk for developing diabetes in an effort to educate young people who live there on how and why they can reduce their risk.  He talked about how these communities tend to be “gymnasium deserts”, having few if any facilities that would allow kids to be physically active in a safe place.  He went on to share how these communities also tend to have limited availability of quality whole foods that are healthy choices that decrease risk of obesity and are more heart-/diabetes-friendly.

Our discussion focused on how the Diabetes Prevention Planning Team is working to pull more resources together to help reduce the rate of new diabetes patients in our communities.  We talked about how public service announcements are being put together by the county government, and how their team needs access to funding to support web development, additional PSA information, and more manpower to help handle some of the work necessary to grow the network of support/collaboration.  We hope our listeners share this information so that we can have a significant impact on this disease that is costly in its effect on our community’s health as well as financially.

During this informative session, Dr. Darvin Hege, Psychologist, with Sean Vojtask, Executive Vice President of Operations and Deborah Frazier, HealthCare Executive, both with BlueWave Computing, discuss the importance of Healthcare IT… all brought to you by HealthGate.

Each day amazing new technology tools are being released that can help practices increase revenue, improve proficiencies, reduce costs and improve patient outcomes. For this reason, the dependency on healthcare technology is rapidly changing. With this comes new software tools, mobile devices, telemedicine, connection into HIE’s, new HIPAA/HITECH/OMBNIBUS rules, and reformed payment models – each with a dependency on each other. The foundation to make all of this data secure, and provide reliable access, rests on the network infrastructure itself.
