Every person that has ever spent a day as an in-patient in a hospital has experienced a care transition.  And every person that has stayed in a hospital in the last several years has seen changes in the way in-patient stays and discharges are handled.  One in six of Medicare beneficiaries discharge from a hospital were readmitted within thirty days during 2009.  More than 1/3 were readmitted within 90 days.  In 2009 alone, the cost to taxpayers for unplanned re-admissions was more than $19 billion!

The good news is that these unplanned re-admissions can be drastically reduced by observing some fairly simple guidelines for post-discharge care.  Join Sam and Brad as they discuss the details of Griswold Home Care’s approach to Care Transitions, and explore what other organizations are doing to help keep discharged patients safe and healthy in their own homes.  This particular session describes in more detail the steps Griswold Home Care is taking to help reduce re-admissions, including specific recommendations for each of the four diagnoses:  Myocardial Infarction, Congestive Heart Failure, Pneumonia and Chronic Obstructive Pulmonary Disease (COPD).

Kim Joens is a Midwest girl, born and raised in the Heartland in Nebraska and Ohio.  Later she moved east to Pennsylvania, attending Slippery Rock University, where she earned a Bachelor of Science degree in Recreation and Resource Management.  She went on to become a park ranger in Coral Springs, Florida for a number of years.  Eventually, she had the opportunity to move back to Atlanta, where she had lived briefly.  This time, she worked for Gwinnett County Parks and Recreation for a number of years, also earning her Masters in Social Work at the University of Georgia.

Kim worked at Gwinnett Medical Center for her internship, then took a full time position.  She began working with the palliative care team there, and felt like she had come home.  As one of her teammates moved on, Kim also found herself with an opportunity to do hospice work full time at Seasons Hospice.  She is a Hospice Care Consultant, and spends her days counseling families on their options, and educating them on the benefits of hospice.  Listen in as she shares her journey, and as she talks about why Seasons Hospice is different!  For more information, feel free to check out their website at www.seasonshospice.org, or call them at (404) 250-4950.

Roger Kurth is not only an Atlanta native, but a rare Atlantan who never left!  After graduating from Georgia State University, earning a degree in Personnel Management, Roger embarked on a career in sales.  Holding a number of varied positions, Roger has sold everything from nails to luggage to machinery.  As the economy turned, Roger decided to take matters into his own hands, starting a business that allowed him to take his part time passion for helping people, and making it his vocation.

As Roger researched the market, he noticed there was a particular area in the senior services industry that was under-served:  He discovered a lot of anxiety and trepidation around the transition from a long-established home environment to a more suitable environment for an individual that may no longer be safe staying at home, nor able to maintain their own home.   Furthermore, he realized that most families had no idea how to go about preparing for a move like this – from the physical details like packing and moving, to the emotional details of parting with possessions.

Ann Serrie is a native of New York City.  When she was old enough for the novelty of cold weather and snow to wear off, she promptly moved to Atlanta! J  At the age of 12, Ann’s grandmother told her that she didn’t want to die in a hospital.  She continued to repeat this message for 20 years.  Ultimately, Ann’s grandmother received the benefits of hospice, and died peacefully in her own home.  This forever marked Ann, and caused her to begin volunteering at Hospice Atlanta nearly 20 years ago.  Five years ago, after 13 years of volunteering, Ann was asked to join the Hospice Atlanta staff.  Today she is the Direct of Volunteers at Hospice Atlanta, and loves every minute!

Starting her adult life as a nun in Nebraska, Marilyn Anderson has spent her career in the service of others.  During her service as a nun, she started attending special services in a chapel where older nuns were dying.  This made a huge impact on Marilyn.  Over the course of years, she taught on a Native American reservation in South Dakota, and later earned a Masters Degree in Political Science and International Studies from Notre Dame.  She also earned a Master of Arts degree from the University of San Francisco.  At the age of 54, she met a widower in the Midwest, married and moved to Atlanta.  After formal retirement, Marilyn realized that she couldn’t possibly “just retire”, and started volunteering with St. Jude’s Parish.  This led to volunteering at Hospice Atlanta, where she continues to volunteer today.

Originally from the Florida Panhandle, Sue Sharp is a 27-year veteran of Delta Airlines.  She was repeatedly offered an early retirement plan, and eventually decided to take it and pursue other career interests.  After working as an Executive Director for several non-profit agencies, she experienced a debilitating accident that required her to acquire mobility-related medical equipment.  When she had difficulty finding what she needed, a light went on in her head.

Yohance Parker knows how to communicate!  After graduating from Union College in Kentucky and spending a few years teaching physical education, Yohance entered the business world as a sales representative.   After realizing that he (by his own admission) “wasn’t very good” at it, he started studying the art of communication.   Yohance spent a number of years then working in a skilled nursing facility environment, both in a service and sales role.  During this time, he developed a passion for serving seniors, and particularly in applying his communications skills to this population.  After years of researching, observing and putting what he learned into practice, Yohance saw great improvements in his performance and realized that he could make a difference by teaching others how to be better communicators. Yohance is the Founder of “A Better You” and now coaches groups and individuals on the “Art of Communication” (also the title of his upcoming book due out in January).

You may remember them from our February 17th show, but since then, they have developed an hour long presentation targeted at seniors who have questions about what to do about their home. Combining targeted market research and years of experience, they are helping seniors make educated decisions on a regular basis. Welcome back Dan and John!

Dan Bourland and John Cowan are both Texans! Dan was born and raised in Lubbock, attended Texas Tech, and has lived in the Dallas area for 30 years. John is from Houston, graduating from the University of Houston Business school in 1973. Interestingly, both Dan and John had different careers before deciding to move into real estate. Dan started his career in the Office Products business, and John in the Wholesale Materials business. John had a second career in Outpatient Imaging. While attending the University of Pennsylvania, Mike Bolla and some of his classmates became acutely aware of the need to protect our country’s freedoms as a result of Iranian hostage crisis, and enlisted in the Army.  Consequently Mike has a special place in his heart for veterans, their families, and the unique challenges many of them face.  After selling a successful trust business, Mike decided 10 years ago to give back to these veterans.  At that time, he assisted his father in applying and qualifying for Aid and Attendance benefits – a process that took him 14 months, and a great deal of frustration.  Coincidentally, a local pastor asked him to help a couple of parishioners through the process.  As a result, Mike spent a good bit of time evaluating the process, and streamlining an approach to assist veterans navigate it.  A year ago, Mike decided to expand the reach of his efforts by creating Heritage LifePath to provide a broader array of services to veterans.  Recently, Mike invited registered nurse Nancy Votta to join him in his work.  Mike is thrilled to be able to provide these services free of charge to the veteran.  If you would like more information, or would like to speak directly with Mike, please send him an email at info@heritagelifepath.org, or call (678) 602-9559, or visit their website at www.heritagelifepath.org.
