Hootan Shahidi of ProspHire Joins Patrick O’Rourke to Talk Dental AI, Guided Implants, Revenue Cycle, and DSO Change Management (Dental Business Radio, Episode 64)

Host Patrick O’Rourke welcomes Hootan Shahidi, Senior Manager at ProspHire, for a wide-ranging conversation on dental technology, revenue cycle strategy, and the operational realities facing dental practices and DSOs. Hootan shares what he is seeing at industry events across the country, including the growing use of guided implant workflows, 3D printing, AI-supported imaging, ambient clinical notes, and prompt-based reporting inside dental practice management systems.

The conversation starts with how new clinical workflows can reduce patient visits, improve predictability, and help practices communicate value more effectively. Hootan explains why convenience and time matter to patients and why practices should be clearer about what technology does for the patient experience, not just the clinical result. He also raises a useful operational point: case acceptance is not the same as case completion, especially in larger or more comprehensive treatment plans.

Patrick and Hootan also discuss the revenue cycle implications of AI and better documentation. From image verification to automated clinical notes, the opportunity is not just faster work but stronger claim support, better narratives, fewer denials, and less time spent chasing preventable problems after the fact.

The second half of the episode turns to DSO change management, software conversions, clinical implementation, and the hidden cost of adding major projects to people who are already overburdened. Hootan explains why successful implementation depends on communication, team buy-in, and dedicated support, not just a directive from leadership. Patrick closes the conversation with observations from the insurance side, including payer scrutiny, treatment-pattern outliers, audits, standard fees, and why practices need to take fee strategy more seriously.

Dental Business Radio is underwritten and presented by Practice Quotient: PPO Negotiations & Analysis and produced by John Ray and the North Fulton affiliate of Business RadioX®.

Key Takeaways in This Episode

  • Guided implant workflows and 3D printing are changing how practices think about patient convenience, treatment timelines, and ROI.
  • Practices should communicate time savings more clearly, because fewer visits can be a major factor in patient decision-making.
  • Case completion deserves more attention as a KPI, particularly for implant and multi-phase treatment plans.
  • AI tools in dental software can help improve X-ray quality, documentation, reporting, and claim support.
  • Better documentation can move practices from denial management to denial prevention.
  • Prompt-based reporting may reduce the learning curve for dental practice management systems and make operational data easier to access.
  • DSOs often underestimate the change management required for software conversions and clinical rollouts.
  • Adding major implementation work to already overloaded internal leaders can create burnout, turnover, delays, and hidden costs.
  • Successful change requires communication, participation, and buy-in from doctors, hygienists, office managers, regional leaders, and front-desk teams.
  • Patrick warns that AI and payer analytics are increasing scrutiny around coding patterns, standard fees, and audit risk.

Kevin Johnson on Building an Oral Surgery Practice That Grows Without Burning You Out (Dental Business Radio, Episode 63), with host Patrick O'Rourke

Kevin Johnson, Leverage Consulting, on Building an Oral Surgery Practice That Grows Without Burning You Out (Dental Business Radio, Episode 63)

Host Patrick O’Rourke welcomes Kevin Johnson, founder and CEO of Leverage Consulting, for a live, on-location conversation recorded in Orlando during the Florida Society of Oral and Maxillofacial Surgeons meeting. Kevin shares how he works with oral surgeons and high-performing dental specialists to move beyond surface-level practice management and build practices that support both profitability and quality of life.

The discussion explores why many practices stall despite strong clinical skills and how issues like inconsistent scheduling, weak new patient processes, overwhelmed practice managers, and lack of team accountability quietly limit growth. Kevin explains why real improvement starts with clarity of purpose, disciplined focus on a few priorities, and creating systems that allow the team, not just the doctor, to drive consistent results.

Kevin also addresses the personal side of practice ownership, including burnout, work-life imbalance, and the unique challenges faced by married entrepreneurs and partners working inside the same business. The conversation highlights why strategic planning, communication, and intentional leadership are essential to building a practice that works as an engine for both financial success and a sustainable life outside the office.

Dental Business Radio is underwritten and presented by Practice Quotient: PPO Negotiations & Analysis and produced by John Ray and the North Fulton affiliate of Business RadioX®.

Key Takeaways in This Episode

  • Practice growth requires more than clinical excellence. Strong production comes from systems, accountability, and team alignment, not just surgical skill.
  • The team, not the doctor alone, creates consistency. Scheduling, new patient processes, and role clarity allow practices to move beyond feast-or-famine production days.
  • Burnout is often a leadership and structure problem. Without clear priorities and boundaries, even high-performing practices can drain energy and purpose.
  • Focus beats overload. Progress accelerates when practices commit to a small number of high-impact priorities instead of chasing dozens of fixes at once.
  • A healthy practice supports a healthy life. Financial success, time away from the office, and personal fulfillment are connected, not competing goals.

Patrick O’Rourke, the host of Dental Business Radio, welcomes dental industry veterans Colleen Huff, Kevin Henry, and Tracy Butler for a preview of Insurance Extravaganza 2026, the national conference focused on the dental revenue cycle. The guests discuss what makes the conference unique, including its focused learning environment, hands-on courses, and real access to speakers who stay involved throughout the event. They share why insurance knowledge can no longer sit with one person at the front desk and how communication, personality awareness, and stronger team collaboration can improve the patient experience and practice performance.

Insurance Extravaganza 2026 will be held February 27 and 28 in Orlando, Florida. Protecting Dentists and Their Practices, Jack McKinney, TDIC (Dental Business Radio, Episode 61)

Patrick O’Rourke welcomes Jack McKinney, Vice President of Sales and Service for The Dentists Insurance Company (TDIC) and TDIC Insurance Solutions, for a practical conversation about how dentists can protect their livelihoods before problems strike.

Jack explains how TDIC’s carrier and agency work together to give dentists comprehensive protection, from professional liability and property coverage to cyber, disability, and long-term-care plans. He also shares why dentists should never treat insurance like a one-time purchase, how business model changes such as teledentistry can affect coverage, and why TDIC’s peer-review process for nonrenewals is so unusual in the industry.

This episode is a reminder that running a successful practice isn’t just about clinical skill. It’s about understanding risk, staying proactive, and partnering with advisors who know the dental world as well as you do.

Dental Business Radio is underwritten and presented by Practice Quotient: PPO Negotiations & Analysis and produced by John Ray and the North Fulton affiliate of Business RadioX®.

Key Takeaways in This Episode

  • By dentists, for dentists: The Dentists Insurance Company (TDIC) was created by the California Dental Association to address rising malpractice costs and now provides a full range of coverage options.
  • Two entities, one purpose: TDIC is the carrier, while TDIC Insurance Solutions acts as the agency distributing TDIC and other carriers’ products to ensure dentists have the right fit for their practice.
  • Comprehensive protection matters: Beyond professional liability, practices should consider property, cyber, EPLI, workers’ comp, disability, and long-term care coverage to fully protect their business and income.
  • Be proactive, not reactive: Dentists should schedule regular insurance reviews, especially before acquisitions, relocations, or business model changes like teledentistry.
  • Unique peer review process: TDIC’s nonrenewal appeals are reviewed by a board of dentists, allowing for real-world context and fairness that’s rare in the insurance industry.

Confidentiality Agreements

Confidentiality Agreements in Dental Insurance Company Contracts, with Patrick O’Rourke, Host of Dental Business Radio

Patrick O’Rourke, the host of Dental Business Radio, addresses issues to be aware of when reviewing dental insurance company contracts. He mentions language in confidentiality agreements that has the potential to be a problem.

Dental Business Radio is underwritten and presented by Practice Quotient: PPO Negotiations & Analysis and produced by the North Fulton studio of Business RadioX®.

TRANSCRIPT

Patrick O’Rourke: [00:00:01] Hi. Patrick O’Rourke here, and another installment of contract provisions in Network Participation Agreements that you should be aware of.

Patrick O’Rourke: [00:00:12] The confidentiality portion is reasonable, right? Typically, you know, they have their proprietary information. They have agreed to certain fees for you and you have agreed to keep them confidential. Now, there’s been a lot of real dangerous language put in there about, you know, irreparable harm and all kinds of other, really, language that can be used purposely and intentionally in a court of law to bankrupt you or to get you to settle. A bit one sided if it’s not mutual.

Patrick O’Rourke: [00:00:58] I’m not a lawyer, by the way, and neither is my opinion a substitute for qualified legal counsel licensed to practice law in your state. This is just one man’s opinion. Somebody who happens to read these contracts a lot and have been doing so for over two decades.

Patrick O’Rourke: [00:01:15] So, confidentiality, good, when they try to prevent or if the contract – not they – the contract language tries to prevent you from accessing the expertise of a subject matter expert, you know, consultant, attorney, anybody else who has expertise in these. That’s when I kind of raised an eyebrow. Again, is it mutual? Are you telling me that that particular payor or that network does not use network or consultants, outside business consultants, folks like myself, or anyone else? There’s a number of different consultants out there. They don’t use attorneys. They don’t use CPAs. I would find that very difficult to believe.

Patrick O’Rourke: [00:02:07] So, for what it’s worth, confidentiality, important. Access to others and to advisors, raising an eyebrow. So, thank you for joining me. Hope you have a terrific day.
